If you’re applying for a U.S. Green Card or adjusting your immigration status through USCIS, you’ll need a mandatory immigration medical exam. At Global Travel Clinics in Houston, Texas, we offer U.S. Immigration Medical Exams (commonly called “US Physicals”) conducted by USCIS-designated civil surgeons.

What’s Included in the Exam (Labs & Vaccines are not included in $ 250.00)
- Physical exam by a USCIS-authorized physician
- Review and update of your vaccination records
- Tuberculosis (TB) testing (IGRA blood test or chest X-ray)
- Required blood and urine tests (e.g., syphilis and gonorrhea screening for applicants 15+)
- Completion and sealing of Form I-693 for USCIS submission
What to Bring
- Government-issued photo ID (passport, driver’s license, or similar)
- Your vaccination and immunization history
- USCIS case number, if applicable
- Any prior TB test results or medical treatment documentation
- (Optional) Form I-693 if pre-filled by your lawyer
How Much Does the U.S. Immigration Medical Exam Cost?
The cost of the U.S. Immigration Medical Physical Examination includes the following:
- Basic Immigration Medical Physical Examination – $250
- IGRA Test (TB blood test) – $150
- Gonorrhea Test – $60
- Syphilis Test – $50
- Vaccination status verification
- Copies of your laboratory results
- Certified and Sealed I-693 Form
Additional tests listed below are not included in the base price and are available for an additional fee. These may be required based on your medical history or USCIS requirements:
Vaccine Titers (Antibody Tests):
- MMR – $150
- Varicella – $145
Vaccinations (if required):
USCIS requires proof of the following vaccinations:
- COVID-19
- Diphtheria
- Influenza (current flu season)
- Hepatitis A & B
- Measles, Mumps, and Rubella (MMR)
- Meningococcal
- Pertussis
- Pneumococcal
- Polio
- Rotavirus
- Tetanus
- Varicella
If you are missing any of these vaccinations, our clinic can administer them at an additional cost during your appointment.
Payment Information: Payment can be made by cash or credit card. We do not accept insurance for any services related to Form I-693.
After your Exam:
- The civil surgeon will certify and seal Form I-693 in an envelope for submission to USCIS
- Do not open the sealed envelope, it must be submitted as-is with your immigration application
- You will also receive a personal copy of your lab results after consultation with the doctor
- Please note: The completed medical exam is valid for up to one year from the exam date
